EMS Plate Program

 How to Order your Maryland EMS Plates MDEMSP and NAEMT Memberships are separate.

  • Here you can print your EMS License Plate Request form send it in with your check or money order payable to MDEMSP Inc. send it to:

  2. Dual Memberships with MDEMSP & NAEMT Plates Included. 
  3. Student Memberships No MDEMS Plate.
  4. Retired Membership EMS Plates Included MDEMS Plate Included.
  5. Retired EMS Members No EMS Plates.

Make your check payable to MDEMSP Inc. processing takes about 1-2 weeks once we receive your completed application and process it.
  • 2. You will be mailed an Motor vehicle form you must complete all sections. 
  • 3. The Motor vehicle Administration will mail out your EMS Plates and new stickers if for some reason you do not receive your stickers or some other issue occurs you will have to contact the DMV for assistance.
  • 4. Please remember the MDEMSP can not offer refunds on the EMS Plate Program.
